Rebecca DelleFave BSN, RN, MS, Vice Preseident, Care Management and Integrated Care Delivery, Rochester Regional Health
Rebecca DelleFave, BSN, RN, MS, is an accomplished nurse leader with more than 20 years of nursing experience and a strong record of advancing clinical practice, care delivery, and nursing leadership across complex health care settings. She earned her undergraduate degree from Nazareth College of Rochester in 2005 and her Master of Science in Leading Health Care Systems from the University of Rochester in 2011.
Rebecca currently serves as Vice President, Care Management and Integrated Care Delivery at Rochester Regional Health, where she oversees inpatient care management and social work, ambulatory care management, and supports clinical workflow oversight within the clinically integrated network. In this role, she helps advance integrated, patient-centered models of care across the continuum while supporting clinical practice, operational alignment, and high-value care delivery. Previously, Rebecca served as Vice President and Chief Nursing Officer for Ambulatory Services at Rochester Regional Health, overseeing clinical practice for more than 2,000 licensed and unlicensed clinical support team members.
She has been recognized by the Rochester Business Journal as a Health Care Hero in the categories of Health Care Staff in 2017 and Nurse in 2021. She has also published innovative nurse leadership models and ambulatory nurse-sensitive indicators. Rebecca co-led the AAACN task force on Ambulatory Nurse-Sensitive Indicators, which completed and published an updated industry report. In 2022, the ambulatory organization she supported achieved its initial Magnet designation from the American Nurses Credentialing Center. She resides in the Rochester area with her husband and two daughters and has called the Rochester community home for 38 years.
Keely E. Dwyer-Matzky MD, Associate CMO for Patient Flow & Capacity Management, URochester Medicine
Dr. Dwyer-Matzky is an Associate Professor of Medicine and Pediatrics at the University of Rochester and practices as an attending physician in both Medicine and Pediatric Hospitalist services since 2007. She also serves as the Associate Chief Medical Officer for Patient Flow and Capacity Management at the University of Rochester Medical Center. In her leadership role, Dr. Dwyer-Matzky focuses on enhancing the delivery of high-quality care while improving operational efficiencies across the UR Medicine hospital system. She leads the Better Flow program and collaborates with the UR Medicine Consult and Transfer Center to ensure patients are receiving the right care, at the right hospital, at the right time, supporting effective load balancing across the enterprise.
Clare B. Park D.O, Assistant Professor, Hospital Medicine, URochester Medicine
Dr. Clare Park grew up in southeast Michigan, where she completed her undergraduate studies at the University of Michigan and her medical degree at the Detroit campus of Michigan State University College of Osteopathic Medicine. She moved to Rochester for her residency in Internal Medicine & Pediatrics and stayed as an adult hospitalist at Strong Memorial Hospital after finishing her training. She has interest in medical ethics and equity work.
Heather Jackson MS, RN-BC, NE-BC, CNL, Hospital at Home Senior Nurse Manager, URochester Medicine
Heather Jackson, MS, RN-BC, NE-BC, CNL is the Senior Nurse Manager of the Hospital at Home Program at UR Medicine. Hired one year prior to the program’s launch, Heather led nursing recruitment, development of onboarding and training programs, and preparation of the nursing team for implementation and launch. She now leads nursing operations and ongoing program development, with a focus on patient safety, workforce development, clinical operations, and innovation. Heather has over 10 years of progressive nursing leadership experience and holds master’s degrees in Business Science and Nursing.
Keri Hadcock RN BSN CMGT-BC, Senior Director of Community Programs and Initiatives, FLPPS
Keri Hadcock, RN, BSN, CMGT-BC is a healthcare executive with more than 30 years of leadership experience spanning health systems, health plans, government initiatives, and community-based organizations. She currently serves as Senior Director of Community Programs and Initiatives at FLPPS (part of the Combined Entity of Common Ground Health, FLPPS, and Rochester RHIO) where she leads the development and implementation of innovative programs that improve healthcare outcomes, strengthen community partnerships, and advance population health strategies across the Finger Lakes region.
Throughout her career, Keri has held executive leadership positions with FLPPS, Rochester Regional Health, and MVP Healthcare, overseeing large-scale healthcare operations, population health initiatives, care management programs, and strategic transformation efforts. She is recognized for her expertise in program development, operational excellence, data-driven decision making, workforce leadership, and community engagement. A Registered Nurse and board-certified Case Manager, Keri is passionate about building collaborative partnerships that improve access to care and create sustainable solutions for complex healthcare challenges.
